Two long weekends: Thanksgiving Thursday through Sunday, and Christmas Friday through Sunday followed by a short week into a New Year's Friday. For a cold-chain package, a long weekend is the problem. A box shipped overnight on a Wednesday before a Thursday holiday can sit in a depot until Friday or Monday.
The refill rule: pull forward, never push back
Pull every refill due in the holiday window forward so the package lands before the closure, not after. Our rule of thumb:
- Due November 24 to 30: ship by Friday, November 20, or hold until Monday, November 30 if the patient has enough supply. Do not ship cold-chain packages on Tuesday, November 24 or Wednesday, November 25.
- Due December 18 to January 4: ship in the week of December 14, with a confirmed delivery by Friday, December 18. That covers Christmas and New Year's in one move.
- Due January 5 to 8: ship Monday, January 4 or Tuesday, January 5, never Thursday, December 31.
Weekly-injection patients usually get four doses per box. A two-week pull-forward means the patient holds a fifth and sixth dose at home for a while. That is a storage question (below) and a cash question: a pulled-forward Zepbound fill also restarts LillyDirect's 45-day refill window, which patients need to keep their lower price. Run the date math before you message anyone.
Check two things with your pharmacy partner this week: its own closure days, which may differ from the carriers', and the last day it will ship refrigerated product before each holiday. The January surge build list asked for this conversation in October; if it has not happened, it is the first call to make tomorrow.
Your refill engine should do the scheduling. A patient who has to ask for an early refill is a ticket; a patient who gets "your December refill ships early on December 14" is not. The workflow is in GLP-1 refill operations.
Cold chain: what the labels say
Your pharmacy decides packaging. You need to know the label so your patient messages are accurate.
Wegovy pens. Store in the refrigerator at 36°F to 46°F (2°C to 8°C). If needed, before the cap is removed, the pen can be kept between 46°F and 86°F (8°C to 30°C) in its carton for up to 28 days. Do not freeze; throw away a pen that has been frozen, exposed to temperatures above 86°F, or out of the fridge for 28 days or longer (FDA label).
Zepbound single-dose pens and vials. Refrigerate at 36°F to 46°F. If needed, each can be kept unrefrigerated at up to 86°F (30°C) for up to 21 days, and should not go back in the fridge once it has been out. Do not freeze. The KwikPen has its own storage terms on its label, so do not copy the single-dose rule into KwikPen messaging.
The Wegovy pill and Foundayo are tablets, so holiday shipping is an ordinary parcel. If you have patients choosing between pill and injection for other reasons, December is not a reason to switch them, and that decision belongs to the clinician anyway.
The December risk is cold more than heat. A box left on a porch in Minneapolis on a Saturday can freeze, and a frozen pen goes in the trash according to the label. Patients in cold states should get a "bring it inside on the day it arrives" message with tracking, and your support macros should cover "it arrived frozen" with a replacement path your pharmacy has agreed in advance.
Patients who travel
A lot of patients will fly in late December. TSA's guidance, in short: medically necessary liquids, including injectable medication, are allowed in carry-on bags in reasonable quantities above the 3.4-ounce limit and should be declared at the checkpoint for separate screening. Medically necessary gel ice packs are allowed whatever their state (frozen, slushy or melted); tell the officer. Check TSA's liquid medication page before you write this up, since the wording has changed over the years.
What we would put in the travel message: keep pens in the carry-on, in the original carton with the pharmacy label; carry a small insulated case; count the days out of the fridge against the label (28 days for Wegovy pens, 21 for Zepbound single-dose); and take only the doses needed for the trip. Crossing time zones with a weekly injection is a dosing question, so the message should send patients to your clinical team, not answer it.
The December support spike
December produces fewer new patients and more questions per patient. Expect "where is my order" to climb as carriers slow, "it was left outside" with every cold snap, "can I get it early, I am travelling" from the week of December 7, and billing questions as the $199 Wegovy intro offer ends on December 31 and patients see $349 in January.
Three messages sent before anyone asks remove most of it:
- Week of November 16: the holiday schedule. Your refill date, whether it moves, and the date it will ship. One message, personalized.
- Week of December 7: travel and storage. The TSA basics, the label's temperature windows, and a "travelling? request your refill date change here" link to the portal.
- Week of December 14: the January price note, only for patients affected by the end of the Wegovy intro price, stating the new amount and the date it starts.
Then staff for the remainder. If you have tagged a month of tickets, the support staffing model gives you hours by category. Schedule clinical coverage across December 24 to January 1 explicitly: side effects do not observe holidays, and a patient who can't reach anyone on December 26 is the one who stops treatment. Write down who is on call for each day, with a named prescriber escalation, before December 1.
Pharmacy status visibility does the most work here. A patient who can see "shipped December 15, arriving December 17" in the app does not write in on December 16.
What Turbopills does here
On Turbopills the refill schedule, pharmacy routing and patient messaging run on the same system, so a holiday pull-forward is a schedule change plus a message, not a spreadsheet. Live order status shows in the branded portal and mobile app, refill reminders and re-authorization prompts follow your protocol, and support sees the same status the patient sees. Your pharmacy partner still owns packaging and its own closures, and your clinicians still own the on-call rota. We are in private beta; if you want to see the refill and status flow before January, ask for it in a demo.
FAQ
What is the last day to ship for Christmas 2026? USPS recommends December 17 for Ground Advantage, December 18 for Priority Mail and December 19 for Priority Mail Express in the lower 48. UPS Next Day Air and FedEx overnight services run to December 23. For refrigerated medication, aim for delivery by December 18.
Are carriers open on Thanksgiving 2026? No. Thanksgiving is Thursday, November 26: USPS and FedEx are closed and UPS has no pickup or delivery. Avoid shipping cold-chain packages on November 24 and 25.
How long can Wegovy or Zepbound be out of the fridge? Per the FDA labels, a Wegovy pen with its cap still on can be kept at 46°F to 86°F for up to 28 days, and Zepbound single-dose pens and vials at up to 86°F for up to 21 days. Neither may be frozen.
Can you fly with GLP-1 pens? Yes. TSA allows medically necessary liquids and injectables in carry-on bags above 3.4 ounces in reasonable quantities, and medically necessary ice packs in any state; declare both at the checkpoint and keep pens in the carry-on.
